When you are planning to have filler, stop taking blood thinners one week before your appointment to avoid bruising and bleeding. Things to avoid include: alcohol, aspirin, ibuprofen (any NSAID), vitamin E or fish oil (any omega-3).

Treating keloids on top of ear

This young lady had keloids on the top of both ears, caused by piercing the upper cartilage of her ears, often called helix piercing.  Unfortunately, her body’s reaction to this piercing was to create excessive scar tissue.

As I often do, I worked on one ear first to see how her body would respond.  I surgically removed the larger of the two keloids, on her right ear (shown far left), and also gave her a Kenalog shot when she came in two weeks later to have her sutures removed.

In the “after” photo (on the right), the right ear is a little more than one month post op and she’s had one injection.

She’s so pleased with the result, she came in today to have the other keloid removed from her left ear.

Keloids result from the overproduction of collagen at the site of an injury.  Keloids cause disfigurement, but also can be painful and itchy.  Keloids have a genetic component.   The good news is that keloids do not turn into cancer.

More than one type of treatment may be necessary to get rid of a keloid.

I recommend taking steps to prevent keloids!

  • If you are considering a body piercing or a tattoo, try a test spot first, wear a pressure earring if you feel a thickening after ear piercing.
  • Tell the practitioner if you have had keloids in the past.
  • Keloids take time to appear, so wear sunscreen to prevent the skin from darkening after a wound.
  • As soon as the wound heals, use silicone sheets or gels which can prevent keloids from forming.

PRP is an injectable made from your blood!

One day after a strong chemical peel.  Yes, I know it’s a bit scary because that’s me!  But how can I recommend it to my patients if I haven’t tried it and experienced the after effects.

All of these photos are from one day after the SkinCeuticals Advanced Corrective Peel.  On the photos where you don’t see red, ISDIN Skin Drops are camouflaging the work of the peel with full coverage.  My face was reddish brown for at least three days.  The peeling started on day three and continued through day seven, with light flaking lasting one month.

I had resolution of my sun spots.  My skin has a smoother, more youthful appearance and a radiant glow that has lasted more than six months.  The results were amazing and in my judgment are as good as a Fraxel laser treatment at less than one third the cost.

I just attended the Swift Beauty masterclass on Facial Injection Anatomy in New York City.

Earlobe Repair

My patient tore her earlobe with an earring.

In the “after” photo you see my sutures that will stay in for 2 weeks.

I re-pierced the ear close to the suture line the same day, so she’ll be ready to wear earrings again. My technique for this works very well.

I treated the keloids on both ears with surgical excision and injections of steroids and other medications every three to four weeks. On the left ear, a pressure earring was applied.  The pressure earring can be helpful by decreasing blood flow to help the keloid from recurring.  I did one ear and waited before doing the other to see how the patient’s body reacted, to ensure a better result.

In the photo, the right earlobe is one month post op and one injection.

The left earlobe is six months out and after six injections.  The pressure earring was used for one month.

Treatment deflates keloid

Keloids are raised scars and if you have one, you might call it a scar gone crazy. They can be painful and also itch. When they grow large on a prominent location on the body, patients’ self-esteem can suffer.

This patient had a large keloid on her chest. We started injections in December and the “after” photo you see is from a few days ago in early May. I chose injections over surgical removal because I felt surgery would risk recurrence, which could be larger and worse.

After her first appointment and injection I suggested she begin using Serica moisturizing scar cream.

As you see from the “after” photo she does have some loss of pigment from the Kenalog injections, but my expectations are that the pigment will normalize with time. As long as there is no trauma to the area, I expect no recurrence of the keloid.

I suggested that she use Obagi Exfoderm on the area to keep the skin cells turning over.
