20% Off Sun Protective Clothing

20% Off Sun Protective Clothing

· Skin Cancer · Comments Off on 20% Off Sun Protective Clothing

BloqUV is a North Miami company that offers sun protective clothing and is giving my patients a 20% discount. I am impressed with the softness of the fabric and how it wicks away moisture. I love the variety of fashionable styles in lots of vibrant colors.

I am not affiliated with the company, nor do I receive any compensation. I do believe sun protective clothing is an important part of protecting yourself from the damaging rays of the sun. In addition to sunscreen, wear protective UPF clothing, a hat and seek out shade, especially from 10 a.m. to 4 p.m.

Prevent Face Mask Irritation

· COVID-19 · Comments Off on Prevent Face Mask Irritation

A recent study found that about half of us who wear face masks will suffer from acne, dermatitis, itch, or pressure injuries.  The likelihood of developing one of these conditions is determined by how long you wear a mask.  Facial skin problems begin to appear with 4 to 6 continuous hours of mask-wearing.

We should continue wearing masks and washing our hands as we enter the flu season.  You can take some preventative measures to avoid irritation from your face mask:

– stick to mild, fragrance-free, and non-comedogenic skincare
– skip makeup (go without foundation, powder and blush)
– just use a light moisturizer and sunscreen on your face if you’re going to be wearing a face mask. I have moisturizers available at my office that also include SPF—great for when you are wearing a mask.
– limit face washing to two times/day and after sweating
– apply moisturizer before and after you wear your mask
– throw away a disposable mask after one day’s use
– cloth masks should be laundered after one day’s use

How the sun affects skin of color

· Sunscreen · Comments Off on How the sun affects skin of color

My patients with darker skin tones also experience the damaging effects of the sun, but sometimes differently than their lighter-skinned counterparts.  The sun may not cause as many wrinkles, but hyperpigmentation and melasma are more of a problem if you have dark skin.

In addition, something called post-inflammatory hyperpigmentation is more of an issue for skin of color.   This is hyperpigmentation that develops at the site of an injury or inflammation that is made worse with sun exposure.  The inflammation can be acne, and patients find the resulting dark spots disturbing.

UV radiation also causes damage in skin of color and can result in skin cancer.  Unfortunately, because of beliefs that dark skin protects one from the sun, patients with skin of color are diagnosed with skin cancer at a more advanced stage.

Because of the propensity for pigmentation, patients with skin of color should select a sunscreen with iron oxides.  Research has shown that using a sunscreen with iron oxides gives greater protection to the block visible light that causes this skin discoloration.  Even though iron oxide might be listed as an “inactive ingredient” on the label, it is providing additional protection if the product has least 2% iron oxide.

You may have to look up your product online to see the full list of all sunscreen ingredients.  Sometimes only the “active ingredients” are listed on the product label.  Iron oxides will not be listed as a sunscreen ingredient.

Tinted sunscreens often have iron oxides and this is why I believe tinted sunscreens are much better for the face.  If your skin is dark and the tinted sunscreen you select is not dark enough, put your foundation on top of the sunscreen.  Also, we are happy to have you test the tinted sunscreens we sell in the office.

Excel V to the rescue!

· Lasers · Comments Off on Excel V to the rescue!

This was a stubborn brown spot.  First, I confirmed it was benign with a biopsy before we began to treat it cosmetically.

It took five monthly treatments with the Excel V laser to get this result.  Both Mylene and I did the laser treatments for this patient.

Once clear, my patient has kept the spot from coming back with Obagi dark spot lightener and corrector cream and a Retinoid.  The “after” photo is 3 years after treatment.  My patient is glad to have it gone and she’s obviously committed to the regimen so it doesn’t reappear.

This diligent maintenance is important!  Otherwise, daily activity, sunlight and environmental pollutants will cause recurrence.

My patient granted Dermatology Boutique permission to post her photos on Dr. Golomb’s social media and patient communications. I am grateful that she allowed me to share her photos to help others.

Fraxel vs. CO2 Laser Treatments

· Lasers · Comments Off on Fraxel vs. CO2 Laser Treatments

I was recently asked about the strength of a Fraxel vs CO2 laser.

CO2 lasers have always been the best choice for fine lines, deeper wrinkles and acne scars.  They remove tissue (ablate) resulting in some tightening as well.

Before and After CO2 CoolPeel Laser

Before and After CO2 CoolPeel Laser

Fraxel is a brand of erbium laser that is also very nice for acne scars and texture.  Fraxel was one of the first fractionated lasers, hence the choice of the brand name “fraxel.”

Fractionated lasers leave areas of tissue sparing therefore there is quicker healing and less chance of side effects.

My CO2 laser is also fractionated, allowing for quicker healing and less side effects such as hypopigmentation (light or white spots).

CO2 lasers can be adjusted in power (depth of penetration) and type of beam.  The Cool Peel technique removes tissue — slender columns with less thermal injury than traditional fraxel.

Healing can be very fast depending on the amount of energy and spacing of energy beams.  I can customize this for each patient, depending on whether they want a light, medium or heavy treatment.

The more energy and closer the spacing (fractionation) the “stronger” the treatment, and hence more down time.

For me there is nothing like CO2 laser!  That’s why I have it.

We also have a Clear+Brilliant laser at my office.  It’s made by the same manufacturer as Fraxel.  It has less power so the recovery is quicker.  It’s a mini Fraxel.

Of course, the choice depends on your skin type and expectations.

The question, which is stronger, is a good one, but they are different devices.

It’s like comparing a BMW to a Porsche.

Both are amazing!

Stem cell derived exosomes are an alternative to PRP

· Exosomes, Hair Loss Treatment · Comments Off on Stem cell derived exosomes are an alternative to PRP

Exosomes rejuvenate and regenerate the skin, just like PRP. Unlike PRP, no blood draw is required.

Exosomes are derived from stem cells and have beneficial healing properties that are especially useful after a heavy laser treatment or microneedling with radio frequency.   Also, Mylene or Bianca can apply exosomes with SkinPen microneedling.  The exosomes accelerate healing time. But even if you haven’t had a treatment, exosomes calm the skin and reduce redness.

With additional applications of exosomes your skin becomes more radiant and hydrated, your skin tone and texture are more even and skin becomes firmer–fine lines, wrinkles and age spots are significantly reduced.

Exosomes can also be used for hair loss by promoting a healthy hair growth cycle.  Exosomes nourish and hydrate the scalp while noticeably reducing hair shedding.

BENEV Exosome Regenerative Complex +  is a post-care “topical use” cosmetic solution.  It is not a drug, nor is it intended to prevent, treat or cure diseases or medical conditions.  It is not intended to be injected or delivered intravenously.

Simplifying Skincare for Men

Some great skincare advice for men from DefenAge…
A man’s skin care routine doesn’t need to be complicated or confusing. Many guys steer clear of a skin care routine for these reasons. But, if you care about the common concerns that most men do—wrinkles, dullness, sun damage, or acne—building an effective regimen is an important step in meeting your skin care goals. A men’s skincare kit can address your various needs. Here’s one simple routine that, if done consistently, can pay off in the way your skin looks and feels over time.

DefenAge men’s skincare kit – available at Dermatology Boutique

Face Cleanser

Ditch the soap if that’s been in your repertoire over the years. Soap is extremely drying and strips your skin of its natural oils and protective barrier. A high-quality face cleanser will practically melt away dirt, sweat, excess oils, environmental stressors, and any other debris on the surface of your skin. A product with prebiotics will keep your skin’s natural barrier healthy and intact. Your skin should be left feeling hydrated and soft, not tight or squeaky clean. A great skin care routine always starts with an effective face cleanser.

Moisturizing Cream

Moisturizers help the skin stay hydrated, but your skin shouldn’t feel greasy when wearing one. The best moisturizers won’t have an off-putting smell either. Choosing a fragrance-free formula is important. A great product will absorb into your skin and you shouldn’t feel like you’re even wearing a moisturizer. With consistent use, your skin can look brighter and feel firmer. The overall tone and texture of your skin can also appear smoother and more even.

Serum

This is like a shot of goodness for your skin. The best serums are potent and designed to absorb effectively into your skin, imparting all the beneficial active ingredients, such as hyaluronic acid and niacinamide. A high-quality serum can help minimize the look of fine lines and wrinkles, revive the skin’s natural moisture barrier, improve the look of visible firmness, and minimize the appearance of skin discolorations. With an anti aging cream and powerful serum, you’re well on your way to great-looking skin.

Exfoliating Mask

If you have two minutes to spare, you can do a weekly exfoliating mask. It’s really that easy—and it makes a huge difference for the overall look of your skin. A great formula can help renew your skin and remove dead skin cells that linger on the surface. The best exfoliating face mask will gently polish and refine the look of your skin with ingredients such as ultrafine sugar crystals and papaya, pumpkin, and pomegranate enzymes, which are natural exfoliators.

Those are the basic products that will help transform your skin, so that it appears smoother, more youthful, and healthier. If you’ve been looking for an ideal way to visibly improve exhausted-looking or damaged skin, start here.

I’m now board certified in Mohs

· Board Certification · Comments Off on I’m now board certified in Mohs

I have great news to share.  I just passed the board certification exam for Micrographic Dermatologic Surgery.  You may know this as Mohs, a type of surgery for skin cancer that is often done on the face to retain as much non-cancerous skin tissue as possible to reduce scarring.

I am now triple board certified in Dermatology, Dermatopathology and Mohs.

This was the first year that board certification was offered for Mohs.  Eventually this certification will be required for a dermatologist to do Micrographic Dermatologic Surgery.

I perform this type of surgery on high-risk or complex skin cancers.  Cancerous tissue is progressively removed and examined until only cancer-free skin remains.  I then repair the area where the cancer was removed.

For example, I performed Mohs surgery on the patient above and then did a skin graft.  She had a basal cell cancer on her ear.  The second photo was taken 3 1/2 months after her surgeries.  Her face mask is pulling her ear forward in the “after” photo.

Mohs is ideal for treating cancer in cosmetically and functionally prominent areas such as the ears, nose, eyelids, lips, hairline, hands and feet.

The exam and board certification demonstrates that I have the knowledge, experience and skills to be a specialist in Mohs.

Does a doctor have to be board certified to practice medicine?  No.

But as a patient you want a physician who stays up-to-date with the latest developments, medicines and treatments in her specialty.  Board certification means your doctor has gone above and beyond the many years of education required for a medical license by passing the exams given by her specialty’s governing body.

Under eye rejuvenation

· Fillers, Injectables · Comments Off on Under eye rejuvenation

RHA filler has a unique formulation that blends well into wrinkles and folds with excellent tissue integration.  The treated areas remain supple so the outcome is natural looking.

Here I used RHA 2 under the eyes.  The “after” photo was taken immediately after injections, so you see a little redness.

The RHA line of fillers includes RHA2, RHA3 and RHA4.  Each version is designed for a different depth of injection depending on what is being treated:  fine lines, folds or volume replacement.

RHA fills in dynamic wrinkles, or wrinkles caused by repetitive facial movements that we make when expressing emotion. Dynamic wrinkles commonly appear around the eyes, cheeks, nose and mouth.

My patient granted Dermatology Boutique permission to post his photos on Dr. Golomb’s social media and patient communications.  I am grateful that he allowed me to share his photos to help others.
