A recent study found that about half of us who wear face masks will suffer from acne, dermatitis, itch, or pressure injuries.  The likelihood of developing one of these conditions is determined by how long you wear a mask.  Facial skin problems begin to appear with 4 to 6 continuous hours of mask-wearing.

We should continue wearing masks and washing our hands as we enter the flu season.  You can take some preventative measures to avoid irritation from your face mask:

– stick to mild, fragrance-free, and non-comedogenic skincare
– skip makeup (go without foundation, powder and blush)
– just use a light moisturizer and sunscreen on your face if you’re going to be wearing a face mask. I have moisturizers available at my office that also include SPF—great for when you are wearing a mask.
– limit face washing to two times/day and after sweating
– apply moisturizer before and after you wear your mask
– throw away a disposable mask after one day’s use
– cloth masks should be laundered after one day’s use

How the sun affects skin of color

My patients with darker skin tones also experience the damaging effects of the sun, but sometimes differently than their lighter-skinned counterparts.  The sun may not cause as many wrinkles, but hyperpigmentation and melasma are more of a problem if you have dark skin.

In addition, something called post-inflammatory hyperpigmentation is more of an issue for skin of color.   This is hyperpigmentation that develops at the site of an injury or inflammation that is made worse with sun exposure.  The inflammation can be acne, and patients find the resulting dark spots disturbing.

UV radiation also causes damage in skin of color and can result in skin cancer.  Unfortunately, because of beliefs that dark skin protects one from the sun, patients with skin of color are diagnosed with skin cancer at a more advanced stage.

Because of the propensity for pigmentation, patients with skin of color should select a sunscreen with iron oxides.  Research has shown that using a sunscreen with iron oxides gives greater protection to the block visible light that causes this skin discoloration.  Even though iron oxide might be listed as an “inactive ingredient” on the label, it is providing additional protection if the product has least 2% iron oxide.

You may have to look up your product online to see the full list of all sunscreen ingredients.  Sometimes only the “active ingredients” are listed on the product label.  Iron oxides will not be listed as a sunscreen ingredient.

Tinted sunscreens often have iron oxides and this is why I believe tinted sunscreens are much better for the face.  If your skin is dark and the tinted sunscreen you select is not dark enough, put your foundation on top of the sunscreen.  Also, we are happy to have you test the tinted sunscreens we sell in the office.

Excel V laser erases stubborn brown spot

This was a stubborn brown spot.  First, I confirmed it was benign with a biopsy before we began to treat it cosmetically.

It took five monthly treatments with the Excel V laser to get this result.  Both Mylene and I did the laser treatments for this patient.

Once clear, my patient has kept the spot from coming back with Obagi dark spot lightener and corrector cream and a Retinoid.  The “after” photo is 3 years after treatment.  My patient is glad to have it gone and she’s obviously committed to the regimen so it doesn’t reappear.

This diligent maintenance is important!  Otherwise, daily activity, sunlight and environmental pollutants will cause recurrence.

Fraxel vs. CO2 Laser Treatments

What is the Difference?

I was recently asked about the strength of a Fraxel vs CO2 laser.

CO2 lasers have always been the best choice for fine lines, deeper wrinkles and acne scars.  They remove tissue (ablate) resulting in some tightening as well.

Fraxel is a brand of erbium laser that is also very nice for acne scars and texture.  Fraxel was one of the first fractionated lasers, hence the choice of the brand name “fraxel.”

Fractionated lasers leave areas of tissue sparing therefore there is quicker healing and less chance of side effects.

My CO2 laser is also fractionated, allowing for quicker healing and less side effects such as hypopigmentation (light or white spots).

CO2 lasers can be adjusted in power (depth of penetration) and type of beam.  The Cool Peel technique removes tissue — slender columns with less thermal injury than traditional fraxel.

Healing can be very fast depending on the amount of energy and spacing of energy beams.  I can customize this for each patient, depending on whether they want a light, medium or heavy treatment.

The more energy and closer the spacing (fractionation) the “stronger” the treatment, and hence more downtime.

The Results

For me, there is nothing like CO2 laser!  That’s why I have it.

We also have a Clear+Brilliant laser at my office.  It’s made by the same manufacturer as Fraxel.  It has less power so the recovery is quicker.  It’s a mini Fraxel.

Of course, the choice depends on your skin type and expectations.

The question, which is stronger, is a good one, but they are different devices.

It’s like comparing a BMW to a Porsche.

Both are amazing!

Board Certified Mohs Surgeon

Dr. Cynthia Golomb: Triple Board Certified Mohs Surgeon

I have great news to share.  I recently passed the board certification exam for Micrographic Dermatologic Surgery.  You may know this as Mohs, a type of surgery for skin cancer that is often done on the face to retain as much non-cancerous skin tissue as possible to reduce scarring.

I am now triple board certified in Dermatology, Dermatopathology and Mohs.

This was the first year that board certification was offered for Mohs.  Eventually this certification will be required for a dermatologist to do Micrographic Dermatologic Surgery.

I perform this type of surgery on high-risk or complex skin cancers.  Cancerous tissue is progressively removed and examined until only cancer-free skin remains.  I then repair the area where the cancer was removed.

For example, I performed Mohs surgery on the patient above and then did a skin graft.  She had a basal cell cancer on her ear.  The second photo was taken 3 1/2 months after her surgeries.  Her face mask is pulling her ear forward in the “after” photo.

Mohs is ideal for treating cancer in cosmetically and functionally prominent areas such as the ears, nose, eyelids, lips, hairline, hands and feet.

The exam and board certification demonstrates that I have the knowledge, experience and skills to be a specialist in Mohs.

Does a doctor have to be board certified to practice medicine?  No.

But as a patient you want a physician who stays up-to-date with the latest developments, medicines and treatments in her specialty.  Board certification means your doctor has gone above and beyond the many years of education required for a medical license by passing the exams given by her specialty’s governing body.

Under eye rejuvenation

Here I used RHA 2 under the eyes.  The “after” photo was taken immediately after injections, so you see a little redness.

Can’t I just take a supplement?

Supplements are a big business, but the manufacturers are not regulated by the FDA. Unless you’ve been guided by a physician to take a vitamin due a deficiency, think twice about supplements.

Diet is the best way to get nutrients. The health of your skin, and even the aging of your skin, is affected by what you eat. Eating a lot of sugary foods on a regular basis can make your skin look older. High-temperature cooking also causes excess glucose in skin fibers and affects your collagen’s ability to repair itself.

Improve your diet with more vegetables, fruits, fish and olive oil. Avoid processed foods and those that are grilled, fried and roasted. Your skin will thank you.
