One appointment per person, please. This way, we can ensure enough time is allotted for your treatment.
Please select “self-pay” for any cosmetic appointments. Cosmetic treatments, such as injectables and lasers, are not covered by medical insurance.
If you want Botox and Filler, please select “Filler & Botox” to ensure enough time is scheduled for both types of injections.

If you have been seen by any provider in our office, you are an “established patient.” If you have never been seen by a provider at Dermatology Boutique before, you are a “new patient.”
